EMERGENCIES AND AFTER HOURS
One of our physicians is available twenty-four hours a day for emergency care. Should you have a problem that you consider an emergency, call 911 and/or go to the nearest emergency room . After normal business hours and on weekends and holidays, the Lancaster Medical Bureau answers our telephone (717) 544-3626 and will contact the on call physician. In the event that you leave a message with our service and your call is not returned within a reasonable length of time, please call again to make sure that we have received the message. In emergencies that result in a visit to the emergency room, please notify the facility personnel that you would prefer services from Lancaster Surgical Group to ensure our physicians manage your care.

From Lancaster City
Take Harrisburg Pike out of town, we are ½ mile past Park City Mall on the left. At the four-way intersection traffic light at the Lancaster General Hospital Health Campus turn left. We are in the 2104 (Orange) Building on the second floor in Suite 200.
From the West
From Route 30 East, take the Rohrerstown/Millersville Route 741 exit. Turn left off the ramp and make a right at the second traffic light on Sylvan Road. Make an immediate left into the Lancaster General Health Campus. Go to the other side of the complex as we are in the 2104 (Orange) Building on the second floor in Suite 200.
From Route 283 east, take the Route 741 exit toward Rohrerstown/Millersville, do not bear to the right (toward East Petersburg), but stay straight as you come off of ramp. Turn left at the next traffic light (0.8 miles) at Harrisburg Pike. At the next traffic light (¼ mile), turn right into the Lancaster General Health Campus. We are in the 2104 (Orange) Building on the second floor in Suite 200.
From the East
Take Route 30 west. Take the Route 741 Rohrerstown/Millersville exit. Turn right off of the ramp. Turn right at next light, Sylvan Road, and make next left into the Lancaster General Health Campus. Go completely to the other side of the complex as we are in the 2104 (Orange) Building on the second floor in Suite 200.
From the South
Take 222/272 North to Lancaster. Turn left on James Street which becomes Harrisburg Pike. Travel about 2.5 miles after turning on James Street. Approximately ½ mile past the Park City Mall turn left at the four-way intersection traffic light into the Lancaster General Health Campus. We are in the 2104 (Orange) Building on the 2 nd floor in Suite 200.
From the North
Take the easiest road to Route 30 West. Take the Route 741 Rohrerstown Millersville exit. Turn right at the end of the ramp. Turn right at the next light, Sylvan Road, and make an immediate left into the Lancaster General Health Campus. Go completely to the other side of the complex as we are in the 2104 (Orange) Building on the second floor in Suite 200.

If this is not your initial appointment, please notify us of any changes in your above stated information.
At your office visit, please be prepared to show identification and your insurance card. If your visit requires a referral due to your insurance plan, please have that process completed prior to your visit and, if applicable, bring along a copy of your referral. Our receptionist will ask you to complete a Patient Registration Forms as well as any of the following applicable Medical Questionnaires that will assist us in your care.
Prior medical history, current medications and allergies are some of the information you will need to complete these forms. Please be prepared to pay by cash, check or Visa/MasterCard for office services and related co-payments or deductibles at the time of your visit.
We make a sincere effort to follow our scheduled appointments. Unfortunately patients do not become ill on schedule, operations do not always run on schedule, and other emergencies do occur. Some patient evaluations take an extended amount of time due to the seriousness of some illnesses. When we fall behind, we ask for your patience and understanding, as we cannot give anyone less than our best care. If an emergency arises that will significantly delay your appointment, we will make every effort to inform you.
Surgery:
We will try to schedule your surgery for a date that is agreeable to you and will attempt to have your surgery and/or hospital admission scheduled while you are in the office. However, because scheduling is a complicated process, when necessary, we will notify you after your visit with the date and time of your surgery. Please remember the time of your surgery is subject to change. If you have a procedure already scheduled you may contact the procedure schedulers directly at (717) 544-3630 with questions.
Every effort is made to schedule your surgery in the facility of your choice, taking into consideration insurance, personal needs, equipment, and physician availability. Please check with the facility and the anesthesia group practicing at that facility to determine if they participate with your insurance plan. If either entity does not participate with your plan, and you would like to change the location of your procedure, please contact our procedure schedulers immediately.
PRESCRIPTION REFILLS
Contact the office Monday through Friday from 9:00 a.m. to 4:00 p.m. Please do not wait until you are completely out of medication to call. Try calling when you have a 2 or 3 day supply left, so your doctor will have ample time to re-order your medication. If applicable, please have your prescription number, the name and number of your pharmacy, the name of the medication, and the dosage available when you call. We ask that you notify the office at least twenty-four to forty-eight hours before you need a refill.
BILLING INFORMATION

Our billing/insurance department exists as a courtesy for our patients.
If we are provided with the necessary information, claims for hospital and/or surgical services will be submitted to your Primary insurance carrier for you. In most cases we will submit claims to your supplementary and/or secondary insurance for you. If we do not have all of the information, the claim will be sent to you and you will be responsible for submitting the claim to your carrier for payment.
Even though your insurance policy may cover a portion or all of your medical expenses, the responsibility for payment of fees is the obligation of the patient or responsible party. Your health insurance is a contract between you and your insurance company; therefore it is your responsibility to contact your carrier if there is a problem with your claim. Coverage varies widely and we cannot predict or guarantee what part of our services will or will not be covered by your particular insurance plan. Failure to notify us in advance of applicable insurance coverage may result in your being directly responsible for the fee regardless of insurance contract.
Payment is requested for all office visits at the time of service. If you have questions about our fees, payments, or your insurance claim please do not hesitate to discuss it with us.

Administrative Forms Fee
It is the policy of LSG to charge a $10 fee for the completion of administrative forms. This fee is the responsibility of patient. Examples of these forms are FMLA, disability, life insurance. This fee is intended to cover the time and materials utilized by our staff in completing these forms.
